State your preferences for mental health treatment if you are later unable to consent.

A Mental Health Advance Directive (Fla. Stat. § 394.4598) lets you specify your preferences about mental health treatment — including medications, hospitalization, and crisis intervention — in case you later become unable to make those decisions yourself.

You may also name a healthcare surrogate to make mental health decisions for you.

Must be signed before two witnesses; neither witness may be your mental health provider or a provider employee.
