Florida Living Will

Declare your end-of-life medical wishes per Florida's statutory advance directive form.

A Florida Living Will lets you state your wishes about life-prolonging procedures if you have a terminal condition, end-stage condition, or are in a persistent vegetative state and cannot make your own decisions.

Florida law (§ 765.303) provides the statutory form. The document must be signed in the presence of two adult witnesses; neither may be your spouse or blood relative.

Give copies to your healthcare provider, hospital, healthcare surrogate, and a trusted family member.
